**Issue**: 5 CodeQL "Inefficient regular expression" alerts in test file
**Root Cause**: Test file intentionally uses dangerous regex patterns to verify SafeRegex protection works
**Alert Locations:**
1. Line 48: `const dangerous = '(.+)+$';` - Test case for pattern detection
2. Line 104: `const dangerous = '(.+)+$';` - Test case for blocking
3. Line 213: `const dangerous = /(.+)+$/;` - Test case for safeReplace
4. Line 297: `const slowPattern = '(a+)+$';` - Test case for timeout
5. Line 329: `const complexPattern = /^(([a-z])+.)+$/;` - Test case for complexity
**First Attempt**: Used `lgtm[js/polynomial-redos]` suppression format
**Commit**: `af3bd7b` - "fix(security): Add CodeQL suppressions for intentional ReDoS test cases"
**Result**: ❌ Failed - LGTM is legacy syntax, CodeQL doesn't recognize it

**Corrected Format:**

### Phase 4: CodeQL Config Fix - Attempt 3 (7:35 PM - 8:15 PM)
**Discovery**: The `.github/codeql/codeql-config.yml` file had suppressions but used incorrect syntax!
**Problem in Config File:**

2. **S6535** - Removed unnecessary `\/` escape (line 163)
3. **S6353** - Used `\w` shorthand instead of `[a-zA-Z0-9_]` (2 instances, line 178)
4. **S6606** - Used `??=` nullish coalescing operator (2 instances, lines 635-636)
5. **S4325** - Removed unnecessary `!` non-null assertions (2 instances, lines 715, 723)

### 1. CodeQL Suppression Syntax Complexity
**Problem**: CodeQL suppressions required 3 attempts to get right
**Lessons:**
- **In-code comments**: Use `// codeql[query-id]` NOT `// lgtm[query-id]`
- **Config file**: The `id` field MUST be an array: `id: [list]` not `id: value`
- **Placement**: Suppression comment must be on line immediately before alert
- **Both needed**: In-code comments alone don't work if config file is wrong
