### Issue #1: MEDIUM - Unicode Normalization in types.ts
**File**: `src/telemetry/types.ts`
**Finding**: DMCP-SEC-004 - User input processed without Unicode normalization
**Analysis**: FALSE POSITIVE
- File contains only TypeScript interfaces
- No runtime code or user input processing
- Type definitions compile away
**Fix**: Added suppression rules

### Issue #2: MEDIUM - Unicode Normalization in OperationalTelemetry.ts
**File**: `src/telemetry/OperationalTelemetry.ts`
**Finding**: DMCP-SEC-004 - User input processed without Unicode normalization
- Line 97: UUID read from `.telemetry-id` file
- Line 160: Log content read from `telemetry.log` file
**Risk**: Even though these are local trusted files, defense-in-depth requires normalization to prevent:
- Homograph attacks (visually similar characters)
- Direction override attacks (RLO/LRO)
- Mixed script attacks (Cyrillic/Greek confusables)
**Fix Applied**: Unicode normalization with `UnicodeValidator.normalize()`

**Security Benefit**: Defense-in-depth even for local files
- Prevents tampering if files are manually edited
- Protects against file system corruption introducing malicious Unicode
- Ensures consistent validation across all input sources

### Issue #3: LOW - Audit Logging in OperationalTelemetry.ts
**File**: `src/telemetry/OperationalTelemetry.ts`
**Finding**: DMCP-SEC-006 - Security operation without audit logging (UUID validation)
**Risk**: UUID validation is a security-relevant operation that should be logged for:
- Compliance auditing
- Forensic analysis
- Detecting tampering attempts
**Fix Applied**: Added `SecurityMonitor.logSecurityEvent()` for both success and failure cases

- SUCCESS events (LOW severity): Normal operations, performance tracking
- FAILURE events (MEDIUM severity): Potential tampering or corruption alerts
- All events stored in SecurityMonitor's circular buffer (last 1000 events)
- Available for forensic analysis via `SecurityMonitor.getRecentEvents()`

#### 2. Memory Security Architecture (Issues #1320-1323)
**Status**: ✅ Phase 1 & 2 complete, fully active
**Implementation**:
- **Phase 1** (PR #1322): Background validation integration
- `BackgroundValidator` runs every 5 minutes
- Non-blocking: Memory creation instant (always starts as UNTRUSTED)
- Trust level progression: UNTRUSTED → VALIDATED/FLAGGED/QUARANTINED
- Memory.save() API for persisting updates
- **Phase 2** (PR #1323): Pattern encryption
- AES-256-GCM encryption for dangerous patterns
- PatternExtractor separates malicious code from content
- Encrypted patterns stored with auth tags for integrity
- Decryption only when needed by authorized tools
**Safety Measures**:
- Untrusted content sandboxed with visual delimiters
- Quarantined content never displayed
- Validation happens outside LLM context (no token cost)
- Pattern encryption protects sensitive data

### Open Issues Analysis
Reviewed 50 open issues. **None are release-blocking.**
#### 🔴 CRITICAL (Not blocking v1.9.19)
**Issue #1300: Runtime circular dependency** (EnhancedIndexManager ↔ VerbTriggerManager)
- **Impact**: Architectural debt, potential initialization deadlocks
- **Why not blocking**:
- Existed for weeks without runtime failures
- No user-facing symptoms
- No test failures
- Initialization order currently stable
- **Plan**: Fix in v1.9.20 with proper dependency injection
- **Estimated effort**: 4-8 hours
#### 🟡 MEDIUM (Technical Debt)
**Performance Optimizations** (Not urgent):
- Issue #1291: Memory leak in NLPScoringManager (setInterval cleanup)
- Issue #1292: APICache unbounded growth within TTL window
- Issue #1301: Inefficient O(n log n) random sample (use Fisher-Yates)
**Why not blocking**:
- Performance is acceptable
- No memory exhaustion reports
- Optimizations, not fixes

Most open issues are feature requests:
