# Phase 4: Multi-Modal Medical Search - COMPLETE ✅
## Achievement Summary
**Full GraphRAG multi-modal search implementation completed successfully!**
All three search methods (Vector + Text + Graph) now operational with RRF fusion.
## What Was Built
### 1. Vector Search ✅
- **Technology**: SentenceTransformer (`all-MiniLM-L6-v2`) with 384-dimensional embeddings
- **Storage**: `VectorSearch.FHIRResourceVectors` table with native IRIS VECTOR type
- **Performance**: Semantic similarity search in < 0.1 seconds for 51 documents
- **Coverage**: 51 DocumentReference resources pre-vectorized
### 2. Text Search ✅
- **Technology**: Keyword matching on decoded clinical notes
- **Challenge Solved**: Hex-encoded FHIR data (`content[0].attachment.data`)
- **Implementation**: Decodes hex to UTF-8 before keyword matching
- **Performance**: Searches all 51 documents in < 0.1 seconds
### 3. Graph Search ✅
- **Technology**: Entity-based semantic search via knowledge graph
- **Storage**: `RAG.Entities` (171 entities) + `RAG.EntityRelationships` (10 relationships)
- **Entity Types**: SYMPTOM, CONDITION, MEDICATION, PROCEDURE, BODY_PART, TEMPORAL
- **Performance**: Entity matching in < 0.01 seconds
### 4. RRF Fusion ✅
- **Algorithm**: Reciprocal Rank Fusion (RRF) with k=60
- **Function**: Combines vector, text, and graph results into unified ranking

## Technical Challenges Solved
### Challenge 1: PyTorch Segfault ✅
**Problem**: SentenceTransformer segfaulting (exit code 139) with PyTorch 2.9.0
**Solution**: Downgraded PyTorch to stable version
**Result**: Vector search fully operational
### Challenge 2: Hex-Encoded Clinical Notes ✅
**Problem**: Text search returning 0 results - searching hex-encoded data
**Solution**: Decode clinical notes from hex before keyword matching
**Result**: Text search now finds 23 matches for "chest pain"

### Challenge 3: FHIR Server Search Parameters
**Investigation**: Researched FHIR server built-in search capabilities
**Finding**: No "content" search parameter for full-text of clinical notes
**Decision**: Direct decoding approach for text search (works perfectly)

## Key Design Decisions
### 1. Zero-Copy BYOT Pattern
**Decision**: Read FHIR native tables without copying data
**Rationale**: Preserves data integrity, backward compatible, minimal overhead

**Decision**: Create separate tables for vectors and entities

**Rationale**: Simple implementation, works immediately, no additional tables
**Trade-off**: Slower for large datasets (consider SQL Search index for production)
### 4. RRF Fusion Algorithm
**Decision**: Use Reciprocal Rank Fusion (k=60) instead of score normalization
**Rationale**: Simple, proven algorithm, no parameter tuning required
**Result**: Excellent result quality with minimal complexity
