# MIMIC-CXR Integration Status
## Dataset Overview
**Downloaded**: 1.2 GB / ~450 GB total
- ✅ 111,050 radiology reports (text files) - READY

**Data Quality**: Real ICU patient data from Beth Israel Deaconess Medical Center
## Current Status
### Completed ✅
1. **Synthea Synthetic Data** (50,569 clinical notes)
- Extracted from 593 patient bundles
- Vectorized with OpenAI (text-embedding-3-large, 3072-dim)
- Stored in `VectorSearch.FHIRTextVectors`
- Vector search working (0.43-0.62 similarity scores)
2. **MIMIC-CXR Reports** (111,050 radiology reports)
- Reports extracted from ZIP
- Ingestion script ready: `ingest_mimic_cxr_reports.py`
- Structured parsing: FINDINGS, IMPRESSION, INDICATION, TECHNIQUE
- Table schema: `VectorSearch.MIMICCXRReports`

## Lessons Learned
### IRIS Vectors
- Client libraries report VECTOR as VARCHAR in metadata (misleading)
- Always use `VECTOR(DOUBLE, dimension)` in DDL
- Use rag-templates vector_sql_utils.py for safe queries
- TO_VECTOR() doesn't accept parameter markers
### FHIR Data Encoding
- Synthea: Base64-encoded clinical notes
- Tutorial: Hex-encoded clinical notes
- Always check encoding before decoding
### Schema Design
- IRIS cannot ALTER to VARCHAR(MAX) (stream type) when table has data
- Solution: Drop and recreate table with correct schema
- Always use VARCHAR(MAX) for unbounded text fields from start
- Test ingestion: 997/1000 → Schema fix → 1000/1000 ✅
### Cost Optimization
- Batch embeddings (100/batch) reduces API overhead
- OpenAI for development ($1-5/month)
- NIM for production ($160/month with auto-stop)
- 78% cost savings with smart EC2 scheduling
