## What Went Wrong
Phase 4.5 was deployed with **5 critical bugs** that made the entire visual UI non-functional:
### Bug #1: Template Variable Mismatch
- **Issue:** Route didn't pass `extractions` list to template
- **Impact:** Gallery section hidden (condition `{% if extractions %}` failed)
- **Fix:** Added `extractions_list` creation in `web/app.py`, passed to template
### Bug #2: URL Path Mismatch (CRITICAL - Blocked ALL images)
- **Issue:** Template used `/plan-analysis/*` URLs, routes defined `/plan-images/*`
- **Impact:** Every image request returned 404
- **Affected:** Thumbnails, detail cards, lightbox, comparison panels
- **Fix:** Global replace `/plan-analysis/` → `/plan-images/` (11 occurrences)
### Bug #3: Download ZIP Path Mismatch
- **Issue:** Template called `/download-zip`, route defined `/download-all`
- **Impact:** ZIP download completely broken
- **Fix:** Changed template to match route
### Bug #4: Email Route Path Mismatch
- **Issue:** After global replace, template called `/plan-images/email`, route was `/plan-analysis/<session_id>/email`
- **Impact:** Email feature broken
- **Fix:** Updated route to `/plan-images/email`
### Bug #5: Email Comparison Context Format
- **Issue:** Template sent `comparison:X,Y`, server expected `comparison-X-Y`
- **Impact:** Comparison emails failed to parse context
- **Fix:** Changed template format to match server parsing
### Bug #6: PDF Download (Not Implemented)
- **Issue:** Button existed for Phase 4.6 feature (not implemented)
- **Impact:** 404 errors when clicked
- **Fix:** Commented out button

## Root Cause Analysis
**Why did this happen?**
1. **Parallel development** — Frontend (Wave 4) and backend (Waves 1-3) developed by separate agents in parallel
2. **No integration testing** — Agents used mocks, never tested actual routes
3. **No smoke test before deploy** — Code was committed and pushed without manual verification
4. **Planning ambiguity** — Plan mentioned `/plan-analysis/*` as examples, but backend implemented `/plan-images/*` (generic naming)
5. **Graceful degradation worked too well** — No error messages displayed, features silently failed
**This was NOT a Sonnet vs Opus issue** — This was a coordination failure between parallel agents working from the same plan.
