ddflow_pr_sync

Sync GitHub/GitLab review outcomes into the queue: merged requests complete items, requested changes requeue with feedback, closed requests park for a human, and approved green ones may auto-merge.

Instructions

Ask the forge (GitHub/GitLab) what reviewers did with every request in REVIEW, and record it: a merged request completes its item (and retargets anything stacked on it), requested changes send the item back to the queue WITH the review text, a closed one is parked for a person, an approved and green one is merged when [flow].pr_merge = 'on_approval'. ddflow_next does this itself when [flow].sync_on_next is on. Exit 2 = the forge could not be asked, which is NOT 'nothing changed'.
