| FHIR_SERVER_HOST | Yes | FHIR API host URL, e.g. https://api.medplum.com. | |
| PINECONE_API_KEY | No | Pinecone API key for vector search. May be encrypted. Required for document search features. | |
| FHIR_SERVER_CLIENT_ID | Yes | OAuth2 client ID for the FHIR server. | |
| FHIR_SERVER_CLIENT_SECRET | Yes | OAuth2 client secret for the FHIR server. May be encrypted. |
