### Rewritten
- Replaced v1 `@modelcontextprotocol/sdk` architecture with v2 package split:
- `@modelcontextprotocol/server`

- `@modelcontextprotocol/express`
- Replaced legacy `.tool/.resource/.prompt` usage with:
- `registerTool`
- `registerResource`
- `registerPrompt`
- Replaced `McpError`/`ErrorCode` style with `ProtocolError`/`ProtocolErrorCode`.
