## Issues Found
### 1. README Tool Count Outdated
**Location:** README.md line 3, line 158
**Current text:** "26 MCP tools" / "Available Tools (26 Registered)"
**Actual count:** 32 registered tools
**Missing from README:**
- `inspect_mod` - Get complete mod details by ID
- `list_all_mods` - List mods with filtering
- `search_mods_by_stat` - Search mods by keyword
- `get_mod_tiers` - Show all tiers in a mod family
- `validate_item_mods` - Validate mod combinations
- `get_available_mods` - List mods by generation type
**Fix:** Update README to document all 32 tools and add a new "Item Mod Data" section to the Available Tools table.

**Issue:** Architecture diagram doesn't reflect current codebase structure

## Notes
- All core validation tools work correctly
- The `inspect_keystone` "error" in testing was due to test script using wrong parameter name (`name` instead of `keystone_name`)
- Server initialization is slow (~10 seconds) due to loading multiple data sources
- New mod tools (6 total) are fully functional but undocumented in README
