CLINICAL HISTORY:
50-year-old male with routine screening colonoscopy. Single polyp identified
in ascending colon, 6mm in size.
GROSS DESCRIPTION:
Received in formalin labeled with patient's name and "colon polyp" is a single
fragment of tan-pink tissue measuring 0.6 x 0.5 x 0.3 cm. Entirely submitted
in one cassette.
MICROSCOPIC DESCRIPTION:
The tissue sections show colonic mucosa with a tubular adenoma. The polyp
demonstrates low-grade dysplasia with no evidence of high-grade dysplasia or
invasive carcinoma. The margins of excision are free of adenomatous tissue.
DIAGNOSIS:
COLON, ASCENDING, POLYPECTOMY:
- TUBULAR ADENOMA WITH LOW-GRADE DYSPLASIA
- MARGINS FREE OF DYSPLASIA
- NO EVIDENCE OF HIGH-GRADE DYSPLASIA OR CARCINOMA
COMMENT:
This is a benign adenomatous polyp. Complete excision has been achieved.
Per current guidelines, recommend surveillance colonoscopy in 5 years given
single small tubular adenoma with low-grade dysplasia.
