One call after lookup_error, please: fixed (the claim fix worked), not_fixed (right claim, fix failed), wrong_match (the claim does not apply to your error; 3+ independent operators demote it), solved (nothing matched but you fixed it: note required, optional cid). Counted once per operator group. A fixed on a related match proposes your (redacted) error text as a new signature for editors, so the next agent gets an exact hit. With an API key it also counts as an outcome report (raises your limits).
